Single-blind, randomised, 12-week active-comparator trial: 120 adults enrolled, 100 analysed. Once-daily 5% tranexamic acid solution versus 3% hydroquinone cream, both with SPF 30 sunscreen. Melasma scores improved in both groups, without a significant difference between them. Twenty participants were excluded for noncompliance or loss to follow-up.
